1. For high school students, if you have decided upon a college major, please indicate what area you intend to study. If you have not decided on a major, indicate your areas of interest and the types of classes you would like to take.

2. (All Applicants) Please list any classes you have taken in the visual or performance arts. Please indicate when and where you took the class.

3. (All Applicants) Please list any musical or theatrical productions or visual art presentations in which you have participated or performed during the preceding year. For each performance, production, or presentation, please describe in what capacity you participated (set design, musical performance, role, etc.)

4. (All Applicants) Please list any community service organizations with which you are involved and community or volunteer services which you performed in the preceding year. For each community service you performed, briefly describe the volunteer activity, provide the name and contact information for the individual who supervised your service, indicate the dates of the service, and state the number of hours that you worked on each project.

5. (All Applicants) If you were employed during the preceding year, please list any employers and indicate the number of hours you worked per week.

6. (All Applicants) Please submit a 500 to 600 word essay in accordance with the following:

High school students shall submit an essay in which they discuss the importance of the Arts or discuss an individual who has influenced or inspired the student’s artistic endeavors. The essay may discuss the Arts as they relate to society as a whole, historical impact, or how they affect the applicant as an individual. Continuing students (former CLOC scholarship recipients) shall submit an essay describing their continuing education plans including lessons they have learned during their continuing education, personal insights, how they have used any scholarship funds received, the role that financial aid and scholarships have played in their education, and how their educational plans have been impacted. All essays shall be a minimum of 500 words with a 600 maximum word limit.
